Dog Information:

When arriving at the Biscuit:

Please keep the barking to a minimum in the parking lot, to keep our neighbors happy. Also, please do not allow your dog to relieve itself in our parking area/front yard. Every dog entering our facility is brought to their outside yard to eliminate prior to going into their room. All dogs must arrive and depart on a leash (we will keep the leash until departure). Do not leave your dog in the vehicle unattended.

Service Agreement:

Loomis Bed and Biscuit Inn (LBBI) agrees to provide day care and/or boarding for the dog(s) set forth in the registration form, and to provide other services as agreed to by and between the dog owner and LBBI at the rate agreed upon.

I have read #1 and #2 and agree to the terms

3) Food / Medicine:

LBBI feeds the dog the food the dog owner brings for the dog. The dog owner will pre-package food into individual servings with dog’s name on it. A small fee will be assessed if the owner prefers LBBI to package individual meals into baggies. The dog owner will write the dog’s name on any cans of food. LBBI feeds twice per day unless other arrangements are made. If LBBI is to administer medicine for the dog(s), the dog owner will clearly label the medicine with the dog’s name and the instructions. It is important for these medications to be separate from the food (even if they are to be fed in their food) so LBBI can keep track of the medication and confirm that they have been given with each meal.

4) Health / Vaccination:

Dog owner represents that the dog(s) is/are in good health and have not been ill with any communicable diseases within the last thirty (30) days. Dog owner further certifies that dog(s) have all current vaccinations. LBBI will NOT accept vaccines given by private party. Vaccinations required for dogs are:

• Rabies

• DHPP (Distemper, Hepatitis, Parainfluenza, Parvovirus)

• Bordetella (Kennel Cough) – required every 6 months

Many larger boarding facilities require the kennel cough vaccine every six months due to the potential for infection. We find adhering to this policy greatly reduces the risk. Dogs can still contract kennel cough even when fully vaccinated. Please do not hesitate to ask for more information with any additional questions.

The parties understand and acknowledge that LBBI has agreed to render services as described herein based upon the dog owner’s representation that the dog is healthy and has received all required vaccinations, does not suffer from any disability, illness, behavioral characteristics or conditions which could adversely affect another dog being cared for or by LBBI.

I have read #3 and #4 and agree to the terms

5) Behavior / Damages Liability:

Dog owner assures LBBI that the dog is not aggressive and has not at any time in the past harmed or shown aggression or threatening behavior towards any other person or any other dog, and there is no history of the dog’s untoward behavior as a result of being confined in a small area. Notwithstanding such assurances, if the dog’s behavior results in any injury to persons or property, the dog owner shall bear full responsibility and pay any and all such expenses resulting from such conduct.

Dog owner agrees that LBBI shall not be liable for any loss or damage to the dog by any cause whatsoever, including but not limited to fire, theft or escape of the dog. Dog owner agrees that the dog owner shall remain liable for any damages that the dog may cause, including bites, aggressive behavior, damage to other dogs, or other conduct of the dog, and agrees that LBBI owners and staff will not be liable and release them and hold them harmless from any liability of any kind whatsoever arising from the dog’s attendance at LBBI. Dog owner understands that the dog is in an environment where other dogs shall be located together in a social atmosphere, and that LBBI shall not be liable for any damage caused to the dog by injury or other episodes which may have been caused by or from another animal.

Dog owner understands that LBBI desires to maintain the health, reasonable care and comfort of all dogs within its facility, and to adequately provide for the safe keeping of such dogs in which LBBI has custody. Should LBBI in its reasonable belief or the reasonable belief of its employees or other agents determine that the dog may be in danger or may endanger other dogs by virtue of its conduct, behavior or aggressive propensities, LBBI may at its sole discretion, isolate or keep the dog apart from other dogs. The dog owner entrusts LBBI to use its best judgment in caring for the dog owner’s dog(s). Should any decisions affecting the handling of the dog become necessary as the result of inclement weather or other natural causes relating to such decisions, LBBI, its agents and employees shall use reasonable judgment given the circumstances of the event or other cause.

6) Emergencies:

In the event the dog becomes ill or injured, LBBI shall notify the dog owner. If the dog owner cannot be contacted, or has failed to instruct LBBI regarding measures to be taken, or if an emergency situation exists which requires immediate action, LBBI is authorized to engage the services of a veterinarian, or to administer medicine or give such other care for the dog that appears to be reasonably advisable. LBBI may advance any and all costs of the dog that arise as a result of the injury or illness, including veterinary costs. Dog owner shall reimburse any such advancements immediately upon pick-up of the dog or when the services have been completed.

I have read #5 and #6 and agree to the terms

Aggression:

If LBBI determines that your dog is aggressive to others during their stay, LBBI will provide separate, private housing for your dog at a $10/day surcharge. Since LBBI is not a traditional kennel, it is important that all dogs get along with others.

Barking Dogs:

If we determine that your dog has a constant barking problem, we will notify you about possible alternatives.

If we determine that your dog chews/damages any of our furniture, walls, belongings, etc., we will provide client with a detailed bill for replacement costs, up to $150.

Potty training:

Other than an occasional accident, dogs are expected to be potty trained.

Special Needs Dogs:

We are happy to accommodate dogs who have trouble navigating, are blind, deaf, or otherwise disabled. Please let us know if your pet is special needs so we can accommodate the best way possible for them. Amount charged for any services, including but not limited to those listed above, may be changed at any time and become effective upon posting at LBBI.

Required Daycare:

If a dog has not been to daycare or stayed overnight with LBBI in over a year, a daycare prior to the stay will be required. This greatly increases a dogs comfort level staying away from their home.

Release of Dog: LBBI will not release dog(s) to anyone but the dog owner without the dog owner’s prior authorization.
